Creating Individualized Family Service Plan

Your work on the Project consists of the 3 steps: 1.Collecting data about the family which has a disabled child with the use of survey method and preparing a Family Report-1 2.Collecting data about this family with the use of interview method and preparing a Family Report-2 3.Creating an Individualized Family Service Plan on the basis of Family Reports 1 and 2. The form consists of: 1. Formal/biographical data about a child, a parent/family, UFSP team members (should be submitted as Family Report 1), 2. Informal data about parental beliefs, feelings, ideas, and a child’s development (should be submitted as Family Report 2 ), 3. Your suggestions about outcome activities and development of transition plans Requirements: 1.No blank lines should be left in the form. 2. Attach all relevant materials: Family Report -1, Family Report -2, screen shots of visual interview, questions created for your interview, etc. that are utilized . 3. Check your IFSP on syntax, grammar, and punctuation. MAKE SURE VIII and IX aspects of IFSP are complete, filled out in detail.
