Instructions:
Review the following patient scenarios:
• A 16 year old 16 week pregnant female is present in your office and her doctor wants to ensure everything is fine. Her height is 5’3” and her current weight is 125 pounds. Her pre-pregnancy weight was 115 pounds. She informs you she is always hungry and eats at least 5 times per day. She is a lacto vegetarian and eats a lot of cheese and peanut butter throughout the day. She also eats fruits and vegetables and loves to eat bagels and bread. She also snacks on cheese puffs and ice cream constantly. She used to be on a dance team and still exercises about 1 hour per day. She tends to forget to take her multi-vitamins and does so about every other day.
• A 40 year old 16 week pregnant female is present in your office and her doctor wants to ensure everything is fine. Her height is 5’3” and her current weight is 145 pounds. Her pre-pregnancy weight was 125 pounds. She informs you she is always hungry and eats at least four times per day. She eats meats, fruits, vegetables, starches, and milk. However, she is trying to limit her starch intake. Since this is her third pregnancy, she wants to remain in shape and exercises one hour per day. She takes a pre-natal vitamin daily.
Based on the above scenarios, answer the following questions. Your responses should be phrased as though you were counseling each woman. This means explaining your answers in detail. Please refer to question one for an example of an acceptable response.
1. Are both women gaining the proper amount of weight during pregnancy? Why or why not?
• Example response: Even though you are 16 years old, at this stage in pregnancy you are/are not gaining the proper amount of weight. Based on the pregnancy guidelines, since you were of normal weight (X pounds) prior to pregnancy, you should be gaining X amount of weight during your X trimester. Even though you are 40 years old, at this stage in pregnancy you are/are not gaining the proper amount of weight. Based on pregnancy guidelines, since you were slightly above weight (X pounds) prior to pregnancy, you should be gaining X amount of weight during your X trimester.
2. What are the recommendations for weight gain of an underweight, overweight, and normal weight pregnant female?
3. Does the calorie need differ from the 16 year old and the 40 year old pregnant female? Why or why not?
4. What nutrients need to be increased during pregnancy? Is either the 16 year old pregnant female or 40 year old pregnant female consuming these additional amounts?
5. Are the risk factors the same for a 16 year old and a 40 year old pregnant female? Name at least two risk factors that affect a 16 year old and 40 year old pregnant female.
6. How can proper nutrition and exercise help to reduce risk factors?
7. How does proper nutrition and exercise impact pregnancy outcomes? (Identify two pregnancy outcomes that are impacted with proper nutrition and exercise.)
Both the 16 year old female and 40 year old female have given birth to their babies 3 days ago. The 16 year old female has decided to formula feed her baby as she is in pain when the baby nurses. The 40 year old female has decided to breastfeed her baby for at least 6 months.
